Telehealth Informed Consent
Informed consent for telehealth services with a Wasef Health healthcare practitioner through VIVIE.
Effective Date: September 25, 2026
Thank you for choosing VIVIE LLC (“VIVIE”) to facilitate access to healthcare services provided by licensed healthcare practitioners affiliated with Wasef Health.
You are viewing this consent because you have elected to receive healthcare services through telehealth.
Depending on your location, medical needs, and applicable law, your telehealth care may include asynchronous communication, secure questionnaires, messaging, telephone communication, live audio/video visits, or a combination of these methods.
BY CLICKING “I AGREE,” YOU ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTAND THIS TELEHEALTH INFORMED CONSENT, UNDERSTAND THE POTENTIAL BENEFITS, RISKS, AND LIMITATIONS OF TELEHEALTH, AND VOLUNTARILY CONSENT TO RECEIVE TELEHEALTH SERVICES UNDER THESE TERMS.
1. What Is Telehealth?
Telehealth is the delivery of healthcare services through electronic communications and technology when you and your healthcare practitioner are in different physical locations.
Telehealth may be used for healthcare services including:
- Medical evaluation;
- Consultation;
- Review of your medical history;
- Review of symptoms and health information;
- Diagnosis where clinically appropriate;
- Treatment recommendations;
- Prescribing, where medically and legally appropriate;
- Follow-up care; and
- Other healthcare services determined appropriate by your healthcare practitioner.
Telehealth allows you to access healthcare without necessarily traveling to the same physical location as your healthcare practitioner.
Telehealth is not appropriate for every medical condition or situation.
2. What Is Asynchronous Telehealth?
Asynchronous telehealth, sometimes called “store-and-forward” telehealth, allows you and your healthcare practitioner to exchange health information at different times rather than communicating simultaneously.
For example, asynchronous telehealth may include:
- Completing a medical intake questionnaire;
- Providing your medical history;
- Providing photographs or documents when clinically appropriate;
- Responding to medical questions;
- Secure messaging;
- SMS or other permitted electronic communications;
- Reviewing laboratory information; and
- Receiving messages from your healthcare practitioner.
Your healthcare practitioner may review the information you provide at a later time and respond after completing their clinical evaluation.
“Asynchronous” means that the patient and healthcare practitioner do not necessarily communicate at the same moment.
This differs from synchronous telehealth, which involves real-time communication, such as a live telephone or video consultation.
3. When a Live Visit May Be Required
Although many Healthcare Services may be provided asynchronously, a healthcare practitioner may determine that a live audio/video consultation or another form of evaluation is necessary before providing treatment.
A live visit may also be required because of:
- The state or jurisdiction where you are physically located;
- Healthcare practitioner licensing requirements;
- Telehealth or prescribing laws;
- The particular medication or treatment being considered;
- Your medical history;
- Your symptoms or condition;
- Clinical standards of care; or
- The independent professional judgment of your healthcare practitioner.
If a live consultation or another form of evaluation is required, you will be informed before the applicable Healthcare Service can proceed.
You understand that completing an asynchronous questionnaire does not guarantee that your care can be completed asynchronously.
4. Healthcare Practitioners
Medical services available through VIVIE are provided by appropriately licensed healthcare practitioners affiliated with Wasef Health.
VIVIE LLC is not itself a medical practice and does not independently diagnose medical conditions, prescribe medications, or make medical treatment decisions.
Your healthcare practitioner is solely responsible for exercising independent professional judgment regarding your medical care.
Your healthcare practitioner will determine whether:
- Telehealth is appropriate for your circumstances;
- Additional information is required;
- A live consultation is necessary;
- Laboratory testing is required;
- An in-person evaluation is appropriate;
- A particular treatment is appropriate; or
- A prescription should or should not be issued.
THERE IS NO GUARANTEE THAT YOU WILL RECEIVE A DIAGNOSIS, TREATMENT, OR PRESCRIPTION AS A RESULT OF COMPLETING AN INTAKE OR PARTICIPATING IN A TELEHEALTH ENCOUNTER.
5. Your Physical Location
Healthcare services delivered through telehealth are generally considered to occur where the patient is physically located at the time care is provided.
You agree to accurately identify the state or jurisdiction where you are physically located when requested.
Healthcare Services may only be provided where the applicable healthcare practitioner is legally authorized to provide care.
If you travel or relocate, the availability of telehealth services may change.
You agree to notify VIVIE or your healthcare practitioner if your physical location changes in a way that may affect your care.
6. Potential Benefits of Telehealth
Potential benefits of telehealth may include:
- Convenient access to healthcare;
- Reduced travel;
- Shorter waiting periods;
- Improved access to healthcare practitioners;
- Ability to communicate remotely;
- Easier follow-up care; and
- Ability to provide medical information electronically.
These benefits are not guaranteed.
7. Potential Risks and Limitations of Telehealth
As with any form of healthcare, telehealth has potential risks and limitations.
These may include:
Technology Failures
Electronic communications may be interrupted or delayed because of:
- Internet outages;
- Software or hardware failures;
- Poor data quality;
- Communication failures;
- Device problems; or
- Other technical issues.
Technical problems may delay evaluation, communication, treatment, or follow-up.
Limited Physical Examination
Telehealth may limit your healthcare practitioner's ability to conduct a physical examination or directly observe certain symptoms.
Your healthcare practitioner may determine that telehealth is not sufficient and recommend an in-person examination, testing, or another form of care.
Incomplete Medical Information
Your healthcare practitioner may not have access to your complete medical history or records from other healthcare providers.
If you fail to disclose relevant information—including medical conditions, medications, supplements, allergies, treatments, pregnancy status where applicable, or other relevant health information—there may be an increased risk of:
- Drug interactions;
- Allergic reactions;
- Inappropriate treatment;
- Delayed diagnosis;
- Adverse effects; or
- Other negative medical outcomes.
Privacy and Security Risks
Although reasonable safeguards are used to protect health information, electronic communications may involve privacy or cybersecurity risks.
For example, unauthorized access, technical failures, or security incidents could potentially expose information.
Delays in Communication
Asynchronous telehealth is not continuously monitored.
Messages, questionnaires, photographs, or other information submitted through the Platform may not be reviewed immediately.
TELEHEALTH THROUGH VIVIE AND WASEF HEALTH IS NOT AN EMERGENCY SERVICE AND IS NOT MONITORED 24 HOURS A DAY, 7 DAYS A WEEK.
8. Emergencies
DO NOT USE VIVIE, ASYNCHRONOUS MESSAGING, OR TELEHEALTH MESSAGING FOR A MEDICAL EMERGENCY.
If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.
Do not wait for a healthcare practitioner to review a questionnaire, email, text message, or Platform communication.
9. Accuracy and Completeness of Your Information
You understand that your healthcare practitioner relies on the information you provide when evaluating you through telehealth.
You agree to provide information that is accurate, truthful, current, and complete to the best of your knowledge.
This includes information concerning:
- Your identity;
- Your age;
- Your physical location;
- Your symptoms;
- Your medical history;
- Current and previous health conditions;
- Medications;
- Supplements;
- Allergies;
- Previous treatments;
- Relevant laboratory results; and
- Other information requested by your healthcare practitioner.
You agree to inform your healthcare practitioner if relevant information changes during your treatment.
10. No Guarantee of Treatment or Results
You understand that telehealth may provide convenient access to healthcare, but results cannot be guaranteed.
Your healthcare practitioner may decline to prescribe medication or provide a requested treatment if, in the practitioner's independent clinical judgment, the treatment is not appropriate or additional evaluation is required.
You understand that requesting a specific product, medication, dosage, or treatment does not mean that it will be prescribed.
11. Privacy and Confidentiality
Information collected in connection with your telehealth care will be handled according to applicable privacy and healthcare laws and applicable privacy practices.
Information may be shared between VIVIE, Wasef Health, healthcare practitioners, pharmacies, laboratories, and other authorized service providers as necessary to provide the Healthcare Services you request and as otherwise permitted by applicable law.

Wasef Health or your healthcare practitioner may also provide a separate Notice of Privacy Practices governing Protected Health Information.
You understand that medical information may be disclosed without your authorization where disclosure is permitted or required by law, including in circumstances involving:
- A valid legal or court order;
- Certain reporting requirements involving suspected abuse or neglect;
- Certain public-health requirements; or
- Preventing or reducing a serious and imminent threat to health or safety.
12. Medical Records
Information you submit in connection with your Healthcare Services may become part of your medical record.
This may include:
- Medical intake responses;
- Messages exchanged with your healthcare practitioner;
- Information provided during live telehealth encounters;
- Clinical notes;
- Treatment decisions;
- Prescriptions;
- Relevant photographs or documents;
- Laboratory information; and
- Other clinically relevant communications.
Medical records may be retained for periods required by applicable state or federal law.
You may request access to eligible records in accordance with applicable law and the applicable healthcare provider's privacy practices.

13. Healthcare Practitioner Identification
Your healthcare practitioner will be identified to you in connection with the Healthcare Services they provide.
Depending on the mode of care, identifying information may include the practitioner's:
- Name;
- Professional credentials; and
- Other information required by applicable law.
Information concerning your healthcare practitioner and your clinical interactions may be maintained as part of your medical record.
14. Electronic Communications
As part of your care, VIVIE, Wasef Health, or your healthcare practitioner may communicate with you using contact information you provide.
Healthcare-related communications may include:
- Medical intake communications;
- Requests for additional information;
- Appointment information;
- Consultation notifications;
- Prescription information;
- Laboratory-related communications;
- Treatment follow-up;
- Order or fulfillment information;
- Account notifications; and
- Customer-support communications.
Communications may occur through:
- The VIVIE Platform;
- Email;
- Telephone;
- SMS or MMS;
- Video conferencing; or
- Other permitted electronic communications.
Message and data rates imposed by your mobile carrier may apply.
You understand that ordinary email and SMS may have privacy limitations depending on the technologies used.
Where legally required, additional consent will be obtained before sending certain types of communications.
Marketing communications are governed separately and are not required as a condition of receiving medical treatment unless otherwise permitted by law.
15. Participation Is Voluntary
Your participation in telehealth is voluntary.
You may ask questions concerning telehealth before consenting.
You may decline telehealth services and discuss available alternatives with the healthcare practitioner where appropriate.
You may withdraw your consent to future telehealth services at any time by contacting:
Withdrawal of consent will apply prospectively and will not invalidate healthcare services already provided.
Withdrawal of telehealth consent does not automatically require deletion of your medical record. Healthcare records may need to be retained under applicable healthcare, professional, or legal requirements.
16. Right to Stop or Modify a Telehealth Encounter
You or your healthcare practitioner may determine that telehealth should be discontinued during a particular encounter.
Your healthcare practitioner may recommend:
- A synchronous audio/video visit;
- Laboratory testing;
- An in-person medical examination;
- Evaluation by another healthcare professional;
- Urgent care; or
- Emergency care.
You understand that following such recommendations may be important to your health and safety.
17. Prescriptions
A prescription will only be issued when an appropriately licensed healthcare practitioner determines that it is medically appropriate and legally permissible.
Telehealth does not guarantee access to any particular:
- Medication;
- Dosage;
- Formulation;
- Treatment;
- Pharmacy; or
- Duration of therapy.
Prescribing requirements may differ depending on your location, the medication involved, and applicable federal and state laws.
18. State-Specific Telehealth Requirements
Telehealth and online prescribing laws vary among states and jurisdictions.
Depending on where you are physically located, applicable requirements may include:
- Specific telehealth consent requirements;
- A live audio/video consultation;
- Additional identity verification;
- Specific disclosures;
- Limitations on asynchronous care;
- Specific requirements for establishing a practitioner-patient relationship; or
- Other clinical or prescribing requirements.
VIVIE and Wasef Health may modify the format of your telehealth encounter as necessary to comply with applicable law and clinical requirements.
Healthcare Services will only be made available where the applicable healthcare practitioner is legally authorized to provide them.
19. Your Acknowledgments
By accepting this Telehealth Informed Consent, you acknowledge that:
- You have read and understand this consent;
- You have had the opportunity to review the information presented;
- You understand how telehealth and asynchronous telehealth work;
- You understand the potential benefits, risks, and limitations;
- You understand that telehealth is not appropriate for emergencies;
- You understand that asynchronous communications are not monitored continuously;
- You understand that accurate and complete medical information is important to your care;
- You understand that telehealth may not be sufficient for every condition;
- You understand that a live or in-person evaluation may be required;
- You understand that treatment or a prescription is not guaranteed;
- You understand that your healthcare practitioner exercises independent professional medical judgment;
- You consent to the electronic transmission of information reasonably necessary to provide your Healthcare Services;
- You consent to receiving Healthcare Services through telehealth where determined appropriate; and
- You voluntarily choose to participate in telehealth.
20. Consent
BY CLICKING “I AGREE,” I CONFIRM THAT I HAVE READ AND UNDERSTAND THIS TELEHEALTH INFORMED CONSENT AND VOLUNTARILY CONSENT TO RECEIVE TELEHEALTH SERVICES FROM HEALTHCARE PRACTITIONERS AFFILIATED WITH WASEF HEALTH THROUGH VIVIE.
This consent will remain effective during your treatment unless you withdraw it or applicable law requires renewed or additional consent.
